On occasion we have situations where our users will struggle with something out in the field.  We want everyone to have a spectacular experience.  We also know that everyone is at varying levels of aptitude in the technology area.  

There comes a point when we need much more information than they usually provide.  Especially, When we cannot duplicate an issue in our environment we need more data for what is going on where they are or with what they are doing.

For that reason we have created a document that will help us to troubleshoot your issue.  When filling out the document, please fill out as much as possible.  As much as you know.  Again, the more you can convey to us, the closer it gets us to providing you a solution.
